Holmium laser is a new type of laser produced by pulsed solid laser device made of yttrium aluminum garnet (YAG) as the active medium, doped with sensitized ion chromium (Cr), energy transfer ion thulium (Tm) and activated ion holmium (Ho) laser crystal (Cr:Tm:Ho:YAG)

The application of holmium laser has brought the treatment of urinary calculi to a new level. The holmium laser, with a wavelength of 2.1μm and a pulse laser, is one of the latest lasers used in many surgical procedures. The energy produced can vaporize the water between the end of the fiber and the stone, forming a tiny void, and transfer the energy to the stone, so that the stone is crushed into a powder. The laser used in the past has many disadvantages, such as non-pulsed carbon dioxide laser, it is the use of thermal effect vaporization of stones. Due to high temperature (2000 ~ 3000℃), so the heat damage is large; Pulsed lasers, such as dye lasers, they are ineffective for cystine stones without pigment; Neodymium lasers cannot be used for lithotripsy because of their poor accuracy.

 

Holmium laser has the advantages of precise control on energy transmission and subsequent incision, what's more, small penetration, shallow cutting depth, relatively narrow thermal damage area, limited damage to surrounding normal tissue, and high safety. Moreover, the operation is under direct vision, and the small-caliber optical fiber with various caliber endoscope can easily reach any part of the urinary system, which is an ideal tool for internal incision. It can be used for: (1) internal incision of urethra and bladder neck stenosis; (2) Internal incision of ureteral stenosis; (3) Internal incision of ureteropelvic junction stenosis (UPJ); (4) Internal incision treatment of congenital megaloureter; (5) Internal incision of ureteral intestinal stricture after urine flow diversion; (6) Laser incision of ureterocele in children; (7) The infundibular part of the calyceum is operated with stenosis. In addition, holmium laser can also be used for decompression of uroepithelial cysts, suturing of hypospadias and so on. In short, the holmium laser is a solid-state laser with an emission wavelength of 2100nm, which combines the characteristics of a CO2 laser and a neodymium laser, using the same device to both cut tissue and solidify bleeding. The holmium laser wavelength can be transmitted through optical fiber, so it can be used in endoscopic surgery.
